Investigation Summary
At 8:45 a.m. on July 10, 2018, an employee and a coworker brought a scraper (Caterpillar Model Number 637C-HP4L63) next to another scraper (Caterpillar Model Number 637C FR3P34) to jump start it's dead battery. The employee started the working scraper and then attached the leads of jumper cables to the battery. The employee then climbed onto the front bumper of the scraper with the dead battery to connect the leads to the battery terminals. As the employee opened the battery cover, the employee lost his balance and fell into the engine fan and became caught. The employee attempted to pull his arm free, but was unable. On the second attempt to pull free, the employee lost his balance and fell to the ground as his arm was amputated at the elbow. The employee was hospitalized for two days for treatment of his injury.
