Investigation Summary
At 12:15 p.m. on May 25, 2018, Employee #1, employed by a construction company, was engaged in carpentry work at a multiemployer construction project, a new five-story multifamily residential structure. He was walking on the planks of a scaffold and helping coworkers install third floor trusses when he fell through a second-story window opening, a fall height of 18 feet, and struck his head on trusses stored on the ground.
