29 CFR 1910.27(b)(2)(ix): The employer did not ensure that the ropes of each rope descent system are effectively padded or otherwise protected, where they can contact edges of the building, anchorage, obstructions, or other surfaces, to prevent them from being cut or weakened. Location 100 Summer Street Boston, MA. 02110 a) On October 23, 2023, an employee suffered fatal injuries from a fall while conducting window cleaning operations from a rope descent system where the rope was not adequately protected where it contacted the parapet wall surface. b) On October 23, 2023, employees were exposed to falls where RDS ropes were not protected from damage where they contacted the metal deck railing. The employer High Rise Inc. of which Sky Safety Inc. is a successor corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.27(b)(2)(ix), which was contained in OSHA inspection number 1382113, citation number 1, item number 5 and was affirmed as a final order on 05-28-2019, with respect to a workplace located at 1 Ashburton Place Boston, MA 02108.
