29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: (a) On or about May 28, 2019 - Facility, employees were exposed to struck-by and electrical hazards when the machine specific lockout/tagout procedures for equipment such as, but not limited to, the Pack Mack Bagger, Cutting Vat Dump, and the Bag Take Away Conveyor did not include specific instructions for testing to verify the effectiveness of the energy control measures.
