29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident: On or about June 21, 2024, at approximately 6:30pm, the employer had an employee who sustained injuries resulting in a work-related fatality. On or about June 24, 2024, at approximately 6:30pm, the Cleveland OSHA office learned about the fatality through a media referral.
