29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: Channing oil and gas waste treatment and disposal facility: On or about September 21, 2022, inside the Control Room, the electrical control panel door labeled Centrifuge Water Pump was left open exposing live parts. Employees were exposed to electrical shock hazards.
