29 CFR 1904.29(b)(1): The employer did not enter a one or two line description for each recordable injury or illness. A.) Armormax Refractories, Inc., on or about July 16, 2021 and times prior thereto: Column F for Case No. 1 on the 2021 OSHA 300 log provides the injury and the part of the body affected, but does not provide the object that directly injured the employee. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
