29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). On or about May 17, 2021, an employee was exposed to machine hazards associated with moving parts when they engaged in a service and maintenance activity for replacing a broken harp wire on the Line 11 Slab Cutter. The employer did not ensure that employees implemented energy control application steps prior to allowing them to enter operational areas to remove a broken harp wire that was stuck inside of a cheese block. The machine was still energized and as the employee attempted to remove the wire the machine was actuated and caught their arm between the pneumatic ram and the frame of the harp. Great Lakes Cheese, Inc was previously cited for a violation of this occupational safety and health equivalent standard (1910.147(d)(2) and 1910.147(d)(3)), which was contained in OSHA inspection number 1201705, citation number 1, item numbers 2/a and 2/b, which were affirmed as a final order on March 2, 2017, with respect to a workplace located at 2200 Enterprise Ave, La Crosse, WI 54603.
