On or about 02/13/2025, at 6450 NW 25th Street, Bldg. 704, Miami, Florida, the employer did not verify fall protection training by a written certification while employees from a crew were performing pressure washing duties and another crew removing an existing roof and placing debris in garbage bags in close proximity to the skylights and roof edge while exposed to 25-foot fall hazards when not using any type of fall protection.
