29 CFR 1926.502(i)(4):All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: (a)Lifetime Perimeter Atlanta (North Roof): On or about 2/4/25, the employer exposed employees to fall hazards of 40 feet from the roof through the mechanical shaft in that a cover was not labeled to warn of the shaft when employees were preparing to take measurements of the shaft.
