29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident: The employer is failing to protect employees from the hazards of falling out of the bed of a work vehicle, This was most recently documented on or about July 21, 2022, at a worksite located near Missouri Hwy 13 and SE 640 Road, Warrensburg, Missouri , in which employee died from injuries he sustained as a result of falling off the bed of a moving truck. The employer failed to contact OSHA within 8 hours of the employee's death.
