29 CFR 1910.178(l)(4)(iii): Refresher training and evaluation. An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. On or about February 14, 2022, at the Access Equipment Telehandler Division maintenance shop located at 750 Eastline Road in Kaukauna, Wisconsin, the employer failed to provide required forklift operator evaluations at least once every three years as required.
