29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. At 1921 Jersey Avenue, Green Cove Springs, Florida, on or about January 14, 2022, an employee suffered a caught-in hazard resulting in a hospitalization and the employer did not report the work-related accident to OSHA within 24 hours.
