29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (Standard is applicable to construction per 1926.103) Hastings Utilities - 4520 E. South St. - Hastings NE 68902 - Employer is failing to provide the training information contained in Appendix D of the standard to an voluntarily wearing an employer provided respirator. Employee wore a half-mask respirator during insulation removal at the facility. This violation last occurred on or about the week of November 8 through 14, 2021.
