29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury: On or about 3/19/2021, the employer failed to ensure that the employees utilizing ladders to gain access to tractor trailers, storage areas etc., inspected them prior to initial use in each work shift. Ladders including but not limited to the 20' Werner extension ladder, model #D1120-2.
