29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Number 6 port void tank on the tank barge TB 2901. : The employer failed to report an accident occurring on or about October 4th, 2019 at 09:30 hours in which an employee was setting up to gas weld when the oxygen hose burst causing burns to his abdominal area. The incident was reported to OSHA on October 7, 2019 at 11:30 hours; 74 hours after the accident occurred.
