Text For Citation: 02 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion hazards: (a) On or about September 14, 2011, Kline 5300-11-18H well site near Alexander, North Dakota, approximately four employees were exposed to fire and/or explosion hazards where the employer had not installed an emergency egress or "Geronimo" line. The employee working from the well mast was engulfed by flames where his only means of egress was to climb down the mast directly into the fire. Among other means, one feasible and acceptable means of abatement would be for the employer to correctly install a safety slide or "Geronimo" line in accordance with the best established industry practices, the manufacturer's guidance, and references established by the (API) American Petroleum Institute Recommended Practice 54, Third Edition (Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations), section 6.10 AUXILIARY ESCAPE, subsection 6.10.1: On all land rigs, the derrick or mast shall have an auxiliary means of escape installed prior to personnel working in the derrick. The auxiliary escape route should use a specially rigged and securely anchored escape line attached to the derrick or mast so as to provide a ready and convenient means of escape from the derrick man's working platform.
