29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about May 12, 2024 and at times prior thereto, on the Crazy Dance ride at Adair park in Stilwell Oklahoma, employees were disassembling the Crazy Dance ride for transport without controlling all sources of energy by affixing locks or tags to the energy control devices.
