29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: 800 West Street, Pittsfield, MA: On or about October 12, 2023, employees working within a roadway work activity area were exposed to vehicular struck-by hazards without having been trained by the employer in the recognition and avoidance of unsafe conditions related to work in a roadway, including that an employee was signaling (flagging) traffic through a Temporary Traffic Control zone without having been trained on proper flagging tools and methods.
