29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:(a) (LOCATION) (IDENTIFY EQUIPMENT AND CONDITION(S)) (STATE HAZARD(S) TO EMPLOYEE(S)) a. On or about October 18, 2019, in repair bay 1, the wiring in the distribution box was not provided with an approved cover, exposing employees to 115-volt shock hazards. b. On or about October 18, 2019, in the alcove next to repair bay 1, the junction box was not covered, exposing employees to 115-Volt shock hazards. c. On or about October 18, 2019, at the mezzanine staircase, the junction box was not covered, exposing employees to a 115-V electric shock hazard. d. On or about October 18, 2019, on the mezzanine, the 4-receptacle outlet box was missing a gland on the cable exiting the upper punch-out hole, exposing employees to a 115-Volt electric shock hazard.
