29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: On or about 12/08/2021 at the jobsite where employees were exposed to the hazard of falling through roof openings as they were removing and replacing roof panels. Four employees used the same horizontal lifeline at the same time for their retractable lanyards. The ends of the horizontal lifeline were secured around roof fans.
