29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) International Cushioning Co. on or about February 22, 2022, at the address of 2101 Cedar St. Fremont, OH: The employer did not ensure electrical power to the recycling grinder was isolated and controlled prior to the performance of a maintenance/ service task. Employees unjamming grinder in proximity of the point of operation in this condition were exposed to amputation hazards.
