29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy might be supplied to the particular lines and equipment were de-energized/open, rendered inoperable if possible, and then did not ensure that such means were tagged to indicate that employees were at work: (a) On or about June 29, 2021, two employees installing electrical equipment were exposed to an electrical hazard when the known sources of electric energy that might be supplied to the equipment were not de-energized/open, rendered inoperable if possible, and tagged to indicate employees were at work.
