29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On or about May 28, 2019 - Outside, baler area, employees were exposed to struck-by hazards when operating the forklift with a bent fork, uneven forks, a fork pin that would not latch the fork, a roof/light held in place with shrink wrap, smooth tires not designed for the rough terrain (potholes and gravel), and fluid leaks that had been reported to maintenance but had not been removed from service because it was a rental.
