29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Stretch's Well Service, Inc. at 16266 366th Avenue, Cresbard, SD 57435: On or about and at times prior to October 30, 2018, the employer did not ensure that rotating parts associated with the "Quill/Kelly" located on the George E. Failing water well drilling rig were effectively guarded. As a result of two bolts projecting 2.5 inches horizontally from the rotating Kelly, an employee was seriously injured after being caught-by the powered rotating equipment. This condition exposed the employees to struck-by and caught-by hazards.
