29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and/or was not done in sequence as required by 29 CFR 1910.147(d) (1)-(6): a) Bazzini - On or about August 15, 2019, an employee who suffered a partial amputation to their finger while servicing a clogged electric impeller pump on the M1 Center Holding Kettle, had not performed the proper lockout tagout sequence actions (1) through (6) as specified by paragraph (d). Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
