29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) On or about December 19, 2024, at 3300 NW North River Drive, Miami, Florida, an employee was exposed to caught-between and struck-by hazards when operating a Hyster forklift with damaged seat belts which had not been taken out of service until it was repaired. b) On or about December 19, 2024, at 3300 NW North River Drive, Miami, Florida, an employee was exposed to caught-between and struck-by hazards when operating a Caterpillar forklift with damaged seat belts which had not been taken out of service until it was repaired.
