29 CFR 1910.179(m)(1): A thorough inspection of all ropes were not being made at least once a month. a. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure that the running wire rope/cable associated with the 20-ton hoist on the east Engineered Material Handling overhead gantry crane was thoroughly inspected at least once a month and inspections were documented. The wire rope was used to lift equipment such as, but not limited to, plastic injection molds; employees were exposed to struck-by and crushing injuries in that the wire rope failed when used to lift a mold that weighed 29,150lbs. b. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure that the running wire rope/cable associated with the 20-ton hoist at the west Engineered Material Handling overhead gantry crane was thoroughly inspected at least once a month and inspections were documented. Employees who used the crane's hoist to lift equipment such as, but not limited to, plastic injection molds were exposed to struck-by and crushing injuries. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
