Text For Citation: 01 Item/Group: 004 Hazard:
29 CFR 1926.416(a)(3): Employer did not advise employees of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken: a. Site- On or about August 24, 2022 an employee performing work inside the energized 480-volt main switch gear was not protected from electric shock and burns where the employer did not advise the employee on the hazards involved and protective measures to be taken including, but not limited to; deenergizing the 480-volt main switch gear, utilizing the employer's High Voltage Job Site Analysis form, and implementing the employer's Lockout/Tagout Program. The employee contacted exposed live parts operating at 480-volts and was electrocuted. b. Site- On or about August 8, 9, and 16, 2022 employees performing work inside the energized 480-volt main switch gear and ASCO transfer switch were not protected from electric shock and burns where the employer did not advise the employee on the hazards involved and protective measures to be taken including, but not limited to; deenergizing the 480-volt equipment, utilizing the employer's High Voltage Job Site Analysis form, and implementing the employer's Lockout/Tagout Program.
