29 CFR 1910.146(d)(1): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not implement the measures necessary to prevent unauthorized entry: 194 Cleveland St. Greenfield, MA: On or about January 24, 2025, and at times prior, the employer did not prevent unauthorized entry into permit-required confined spaces when two employees entered a material storage bin in an attempt to rescue an engulfed employee, and were exposed to engulfment and asphyxiation hazards.
