29 CFR 1910.29(g)(2): The employer must ensure cages and wells are continuous throughout the length of the fixed ladder, except for access, egress, and other transfer points; Lindenwood, East grain bucket elevator's vertical fixed ladder cage - on or about April 7, 2020, the employer failed to ensure that cage sections were continuously joined throughout the length of the fixed ladder and had all vertical members in place to ensure intended containment in the event of a fall. Employees ascending/descending on a fixed ladder were exposed to fall hazards up to 100 feet likely to result in death or serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
