29 CFR 1910.303(h)(5)(iii)(B): Permanent and conspicuous warning signs were not provided, reading substantially as follows: "DANGER--HIGH VOLTAGE--KEEP OUT": On or about August 23, 2021, the employer did not ensure all entrances to high voltage rooms containing electrical equipment over 600 volts were marked with appropriate signs. The entrance to the high voltage electrical room containing three phase primary voltage (4KV), located in Building B, US 4 Penthouse substation, was not marked with proper signage, exposing employees to an electrical hazard while storing material in the room.
