Text For Citation: 01 Item/Group: 003B Hazard:
29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a.) 2K Wholesale, Inc., on or about May 26th, 2021 and times prior thereto, a Toyota forklift (model #8FGU30) (Serial #14506) was being used with a dumping hopper attachment. The attachment was used for work operations including, but not limited to, moving scrap wood to the wood bin onsite. The company had not obtained a new forklift plate to identify the attachment and changes in lifting limitations. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
