29 CFR 1917.152(d)(1)(xvi): Oxygen cylinders were not separated from fuel gas cylinders and combustible materials by either a minimum distance of 20 feet (6.1 m) or a barrier having a fire-resistance rating of 30 minutes. a) Worksite: On or about April 8, 2019 an employee was transporting/moving equipment, including an oxygen and acetylene compressed gas cylinders in the back of a GMC Savana van/truck. The cylinders were determined to have contributed to an explosion which occurred while the truck was returning to the laydown yard. The cylinders had not been used in 4 days and should have been placed in storage where they were separated by 20 feet or had a fire resistant barrier between them.
