29 CFR 1926.1203(d): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with �1926.1204 at the construction site: (a) At Adamo Drive and Highway 301 Project, employees were exposed to struck by and carbon monoxide when installing and removing equipment such as but not limited to steel casing supply pipelines, Cherne pipe plugs and compressed airlines to new storm drain systems during leak testing, on or about February 14, 2018.
