29 CFR 1910.120(k)(2)(iv): Appropriate steps were not taken to correct deficiencies in decontamination procedures when they were found to be ineffective: a) Clean Harbors Treatment Building: On or about October 30, 2017, and at times prior, employees working in the treatment building were exposed to dermal injuries from contact with hazardous waste due to lack of appropriate decontamination measures for extremely caustic materials. Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
