Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: a.) 2K Wholesale, Inc., on or about May 26th, 2021 and times prior thereto, a Toyota forklift (model #8FGU30) (Serial #14506) was being used with a dumping hopper attachment. The attachment was used for work operations including, but not limited to, moving scrap wood to the wood bin onsite. The company had not received the forklift manufacturer's approval to use the attachment. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
