29 CFR 1910.269(g)(2)(iv)(C)(1): Each employee working in an aerial lift was not using a travel restraint system or personal fall arrest system: On or about 03/05/2020, Toms Tree Service LLC, located at 1313 S. Main Street, Albia, Iowa, the employer did not ensure that the Bucket Operator at 6957 Weldon Spring, MO, had donned the personal fall arrest harness properly. The leg straps were not engaged while in an elevated aerial lift bucket. An incident occurred with the victim 20 25 feet above ground level.
