29 CFR 19268.1203(a): Before work begins, the employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees are to enter and evaluates any permit required confined spaces: (a) At Adamo Drive and Highway 301 Project, employees were exposed to struck by hazards when installing and removing equipment such as but not limited to steel casing supply pipelines, Cherne pipe plugs and compressed airlines to new storm drain systems during leak testing, on or about February 14, 2018.
