29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) On or about December 19, 2024, at 3300 NW North River Drive, Miami, Florida, two employees were exposed to caught-between and struck-by hazards when operating powered industrial trucks without training and evaluation to operate their forklifts pursuant to 29 CFR 1910.178(l).
