29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): At worksite located at 2915 Nicholson Ave, Kansas City, Missouri. Employees in the warehouse were exposed to crushing, and struck-by hazards. . Employees were operating forklifts, including but not limited to the Toyota forklift Serial number 67003, known to the company as forklift #3. The employer did not develop a site specific forklift program.
