29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: The employer is failing to protect employees from electrocution hazards. This was most recently documented on or about August 19, 2019 at the job site located at SE NorthStar RD and SE Scott Canyon RD, Kiowa, Kansas, when an employee was exposed to energized underground lines while removing an electrical box that supplied power to the decommissioned well site. A lock and tag was not placed on the disconnecting means to ensure the circuit was deenergized and remained deenergized for the duration of the task(s).
