29 CFR 1910.23(b)(11): The employer did not ensure that each employee faces the ladder when climbing up or down it. a) Storage Room, Youth Gymnasium: On January 19, 2021, and at times prior, the employer exposed employees to falls of up to 10 feet in that the Werner extension ladder permanently affixed to the wall across from the landing required employees to turn away from the ladder and cross over a gap approximately 45 cm (1' 5.7") in width to reach the upper level.
