29 CFR 1926.451(e)(6)(i): Integral prefabricated scaffold access frames shall be specifically designed and constructed for use as ladder rungs. a) On December 10, 2020 at 9369 Thoroughfare Road in Minocqua, Wisconsin, an employee was climbing on a top bar of a scaffold end frame eleven and one half feet above surface level to continue climbing down and over to the access/egress rungs. Scaffold near the building's northwest corner did not have end frame rungs constructed for direct access/egress of the work platform. b) On December 10, 2020 at 9369 Thoroughfare Road in Minocqua, Wisconsin, an employee was climbing on the inside of end frames to access/egress a work platform twelve and one half feet above surface level. Scaffold near the building's northwest corner did not have end frame rungs constructed for direct access/egress of the work platform. c) On December 10, 2020 at 9369 Thoroughfare Road in Minocqua, Wisconsin, an employee was climbing on a top bar of a scaffold end frame five and one half feet above surface level to continue climbing down access/egress rungs. Scaffold near the building's northwest corner did not have end frame rungs constructed for direct access/egress of the work platform. d) On December 8, 2020 at 9369 Thoroughfare Road in Minocqua, Wisconsin, an employee was climbing on a top bar of a scaffold end frame greater than ten feet above surface level to continue climbing down and over to the access/egress rungs. Scaffold near the building's northeast corner did not have end frame rungs constructed for direct access/egress of the work platform.
