29 CFR 1926.451(f)(4): Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired. On February 10, 2017, located on the North Wing of area D23, employees were exposed to fall hazards while performing masonry work from a tubular welded frame scaffold with defective adjustable saddle hangers that had not been removed from service prior to employees use of the scaffolding system. Potential fall height measured to be 23.6 feet.
