29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about May 31, 2024 at 19801 Holland Road, Brook Park OH the employer failed to ensure employees operating Powered Industrial Vehicles, to include, but not limited to, a Toyota Forklift Truck model 8FGCU25, Serial number 83948 were trained prior to being allowed to operate.
