29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) At Injection Well 2-2 McDavid Lands, Century FL; On November 18, 2022, employees were exposed to struck-by hazards when performing servicing activities such as, but not limited to, changing the skillets and rings in the piping and a procedure had not been adequately developed, documented, or utilized to control potentially hazardous energy due to water and nitrogen at approximately 5,500 psi.
