29 CFR 1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: (a) Jobsite located 1801 Fairmount Ave. Philadelphia, PA - 19130: The employer did not ensure that the 16ft. portable ladder extended 3ft. beyond the landing surface on the second level of the structure, thereby exposing employees to fall hazards. Condition was observed on or about 18 DEC 2020.
