29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: A.) Penn Stainless Products, Inc., 102 McQuiston Drive, Jackson Center, PA 16133, production area, grinding table, on or about August 13, 2020 and times prior thereto: A yellow flexible cord of a cord reel was powering equipment including, but not limited to, a DeWalt DWE43144N 6-inch angle grinder. Employees were utilizing the angle grinder for various grinding tasks. The yellow flexible cord had a cut in its outer jacket and inner insulation that exposed the bare copper conductor. B.) Penn Stainless Products, Inc., 102 McQuiston Drive, Jackson Center, PA 16133, production area, grinding table, on or about August 13, 2020 and times prior thereto: A yellow flexible cord of a cord reel was powering equipment including, but not limited to, a DeWalt DWE43144N 6-inch angle grinder. Employees were utilizing the angle grinder for various grinding tasks. The yellow flexible cord had a wear mark on the outer jacket that exposed the inner insulated conductor.
