29 CFR 1910.179(k)(1)(ii): The actuating mechanism of the limit switch was not located so that it would trip the switch, under all conditions, in sufficient time to prevent contact of the hook or hook block with any part of the trolley: a. On or about September 18, 2019, on the 125 Log Crane, a calibrated upper limit switch was not installed to prevent contact of the block with the boom tip, exposing employees to the hazard of being struck by parted wire rope and a falling block and grapple.
